  • At the beginning of the episode, there is a big wildfire, but later, it is gone.
  • In some shots during the battle between Four Arms and Malware, Four Arms doesn't have the black stripe on his back.
  • When Four Arms hits Malware with a tree, his Omnitrix symbol briefly appears in his right shoulder instead of his left.

  • In one shot, there is a layering error, where Rook's arm is clipped halfway through his torso.
  • In one shot, the top of Water Hazard's chest is red instead of grey.
  • Outside the gas house, an Alien Vermin is supposed to run up a pipe in the foreground, but its position is offset such that it looks like it runs on air.
  • While Terraspin and Rook are chasing Bubble Helmet, his Omnitrix symbol is colored black.
  • In one shot, Terraspin looks bigger than usual.
  • In various shots, there are duplicates of the Bouncer, Sweet-Eels Sparklefunk, Bug-Lite, and Thunderpig.
  • When Ben is about to transform into Armodrillo, the sound of him looking through his aliens is heard, but the hologram wheel is not shown.
  • In the shot after the one in which Rook says "Ben, behind you", the metal circle on Armodrillo's head is yellow instead of silver.

A Jolt from the Past[]

  • Inside Mr. Smoothy, Young Ben has the remains of a smoothie on him. When he goes out, the smoothie disappears.
  • When Young Ben is about to transform into Feedback, the button to activate the Omnitrix is on both sides.
  • When Rook says that he hopes "the real Ben Tennyson lives up to the legends", his mouth does not move.
  • When Ben activates the Omnitrix to threaten the alien with the handbag, the hologram wheel faces the wrong way.
  • When Ben selects Big Chill, the holographic interface doesn't disappear after the Omnitrix's core pops up.
  • When Ben is hoping to transform into XLR8, Humungousaur's icon is shown.
  • In one shot, the shine on Clockwork's body is darker than usual.
  • Clockwork is missing the black stripes and the keyhole on his back.
  • When the containment device is first shown, it has no Nosedeenians, but in the next shot, it is full of them. This error is seen many times.
  • When Bloxx smashes the ground, he is shown falling for a while, then smashing the ground, but Ben and Rook were only a few feet from the ground. Also, when he does fall, one of his feet is missing.
  • When Bloxx grabs Hoodlum before he can shock Rook, the line on his hand is colored red instead of black.
  • When Ben says "Shutting you down isn't one of them", the Omnitrix symbol on Rook's Proto-Tool is all green and doesn't have the hourglass symbol on it. This error is also seen when the Nosedeenians attack Rook.
  • Ben selects Four Arms' icon. When the camera view changes, it shows Ben's selected alien is now Bloxx.
  • The closed captions on the Ben 10 Omniverse: A New Beginning DVD call Gravattack "Graviton", his concept name.
  • In one shot, Gravattack's Omnitrix symbol is on the side of his head.
  • When Rook gets shot and Gravattack talks to him, the rim of his Omnitrix symbol is the color of Gravattack's skin instead of white.
  • In the interior shot of Fistrick's suit getting flooded, the water leaking inside alternates between the top and lower layer as the lights change color.

It Was Them[]

  • When Ben is about to transform into Diamondhead, there is a faint pencil line on the left side of the screen.
  • In one shot, the soles of Diamondhead's feet are black instead of green.
  • In one shot, Echo Echo's right eye doesn't have its black outline.
  • After defeating the Mutant Ant, the Echo Echo clone's "10" symbol is flipped.
    • The "10" is also incorrectly colored white in this scene.
    • The black lining of the original Echo Echo's headphones is also incorrectly colored white.
  • When Rook splashes goo on Ben's face, the Omnitrix symbol on the Proto-Tool is disproportionate.
  • When Ben and Rook are behind Dr. Animo and Ben calls him, the ant goo is not on Ben and Rook's body.
  • When Ben tells Animo that he can't let him get away with his plan, his fingers are not complete.

Trouble Helix[]

  • When Chromastone reverts back to Teen Ben, some of the laser blasts clip through the wall next to him.
  • In one shot, Cannonbolt is smaller and Blukic and Driba are bigger than usual.
  • In one shot, XLR8's lips overlap the inside of his mouth.
  • During the first shot he appears in, XLR8's outfit doesn't cover the back of his feet.
  • After Azmuth reverts XLR8 back to Young Ben, the Omnitrix's dial is bigger than usual.
  • When Young Ben is about to transform into Feedback, one of the green dots stays on the outer rim of the core instead of being on the dial.
  • Throughout the episode, the Omnitrix's activation button appears on both sides of the dial.
  • When Feedback reverts back to Young Ben, the sound of 16-year-old Ben's Omnitrix can be heard.
  • In some shots, Four Arms doesn't have the stripe on his back.

  • When Ben and Rook walk away from each other after their argument, the Tentacle Vendor is seen standing in a booth, with his feet in the air instead of on the ground.

Of Predators and Prey: Part 2[]

  • Before Young Ben transforms into Stinkfly, his silhouette does not appear on the Omnitrix.
  • In one shot, 11-year-old Stinkfly has nine stripes instead of four.
  • When Rook comes to save Ben by shocking Khyber, the Omnitrix symbol on his Proto-Tool is all green.
  • When 16-year-old Stinkfly reverts back to Ben, the rim around his Omnitrix symbol is grey instead of white.
  • When Crashhopper fights Buglizard, Buglizard's face is entirely black. When it cuts back to her, the white parts on her face are black.
  • Throughout most of the episode, the circle on Rook's back is all black.

  • Before Rath, Gwen and Kevin escape the burning house, Rath's jaw is orange.
  • When Gwen gives Madison her doll back, Gwen's socks are missing.
  • After Madison is saved, her hairclip disappears.
  • Gwen says she is on spring break, even though summer had already passed during Season 3 of Ultimate Alien.
  • Kevin drops his smoothie cup, but when he brings Ben away, the cup is missing.
  • When Kevin is looking at Rook's truck, when Ben says "Um, Kevin", his mouth doesn't move.
  • In one shot, Bloxx has red eyebrows.
  • In one shot, Looma is missing her glove.
  • In one shot, Gwen's shirt is shorter and resembles the shirt that she wears in her Anodite form.
  • In one shot, XLR8's Omnitrix symbol is missing.

Outbreak[]

  • When Dr. Psychobos views schematics for the Omnitrix, the sides of the dial are uneven.
  • When the camera pans out to see the entire room, Zed is gone.
  • When Khyber and Malware recover from Dr. Psychobos' attack, Malware's eye is fully red.
  • Bubble Helmet has blue skin in both his Gimlinopithecus hybrid form and his regular form, instead of pink.
  • In one shot, The Worst's eyebrows are gone.
  • In one shot when Bubble Helmet fights the Plumbers, he has pupils, even though he didn't before.
  • When Liam holds The Worst, The Worst's left leg is missing for a shot.
  • When Bubble Helmet reverts to his regular form, he does not have his helmet on but does not have a hard time breathing, despite it being shown in The More Things Change: Part 2 that he cannot survive on Earth without his helmet.
  • During Kickin Hawk's fight with Liam, Blukic, Driba and Jerry are smaller than usual.

Blukic and Driba Go to Mr. Smoothy's[]

  • After Blukic and Driba rewire the Mouse Minions to attack each other, when they are attacking each other there is one mouse with a closed head. However, since Blukic and Driba rewired them, all of their heads should have been opened.

Malefactor[]

  • The closed captions on the Ben 10 Omniverse: Heroes Rise DVD misspell "Olde Bellwood Days" as "Olde Belwood Days".
  • When Ben and Rook enter the Olde Bellwood Days festival, the sign says "Olde Belwood Days".
  • In one shot, the top of Stinkfly's left wing is cut off.
  • When Ben and Rook look at Buglizard, the tennis balls are bigger than they should be.
  • In the flashback, the Rust Bucket 2 has its predecessor's appearance in some shots.
  • After Max gives the visor to Young Ben, there is an extra frame of Young Ben's head.
  • When Malware absorbs the Plumber rifles, the two circles are black with a red middle on his left arm.
  • In the flashback, immediately before Ben selects XLR8:
    • The button on the Omnitrix is on the wrong side.
    • The dial doesn't open up, only the core.
    • The Omnitrix's core angle is off. It is upright when it should be somewhat tilted on its right (otherwise, some of the core would be visible when the dial isn't popped up).
  • When the Omnitrix times out and XLR8 reverts back into Ben, 16-year-old Ben's Omnitrix time-out sound is heard.
  • In one shot, Big Chill's Omnitrix symbol is not drawn.
  • When Big Chill and Hypnotick reach Billions Tower, Nemesis Tower is not next to it.

Bros in Space[]

  • When Rook greets his siblings, the door to his house is open, but a few shots later, it is closed.
  • While Rook chases the Muroids around the house, he pulls out his Proto-Tool, but when the scene changes, the Proto-Tool is on his back, and after that he is holding it again.
  • When Rook Shim and Rook Shi are cheering for Ben when he transforms into Bloxx, their arms are disconnected from their shoulders.
  • In multiple shots, Shocksquatch's fists are yellow instead of their usual grey.

Arrested Development[]

  • Humungousaur's size is inconsistent.
  • When Billy wakes up, Ben's sneakers are entirely white.
  • When Young Ben transforms into Diamondhead, Diamondhead's face is briefly seen on his chest.
  • When Billy uses the De-Ager ray on Ben and Rook, the Omnitrix is missing.
  • In one shot, Mazuma's shoes are miscolored.
  • In one shot, the back of Bloxx's neck is entirely yellow.
  • In one shot, the portal is not visible.
  • When Billy falls over, he is seen with pants instead of shorts

Rules of Engagement[]

  • When XLR8 runs, his Omnitrix symbol is missing.
  • When Armodrillo and Rook run towards Looma, Armodrillo has no tail.
  • When Armodrillo tickles Looma with his drill, the grey on his head is yellow.
  • In one shot, the green "10" on the Tenn-Speed is black.
  • In one shot, Looma's ship, Tenn-Speed, and the car are in different positions.
  • During the second fight scene with Looma, Rook doesn't have his Proto-Tool with him. Right as Ben and Fistina pull up to his house on the Tenn-Speed, Rook has the Proto-Tool with him.

Showdown: Part 1[]

  • In one shot before Ben puts his arms behind his head, the Omnitrix is missing from his arm.
  • Before 11-year-old Ben transforms into Feedback for the first time, the Omnitrix's activation button is on the wrong side.
  • When Heatblast reverts back to Ben, the shadow underneath him is covered in green light.
  • For a brief moment when Humungousaur steps on a display, his briefs disappear. His sash and Omnitrix symbol have also shifted, being off center and closer to his right shoulder.
  • In one shot, the Nemetrix is completely white on Tyrannopede.
  • While Rook is talking to Azmuth, the symbol on his back is all black.
  • When Omnivoracious is about to swallow Azmuth, her left is cut off due to a rendering error.
  • When Vicetopus lets go of Dr. Psychobos, the tip of her tentacle is the same color as Dr. Psychobos' pincer.
  • During the flashback right as Ben enters the Rust Bucket 2, to the left, a log, a campfire, and rocks can be seen. In that shot, the log is furthest to the right. Right after Malware blasts the Rust Bucket 2, the log is now between the campfire and the rocks.
    • In the aforementioned scene, 11-year-old Ben appears to be colored identically to his 16-year-old self, with a black shirt and brown pants instead of the usual white and black shirt and olive-colored pants.
  • When Ben turns into Feedback to fight Malware, Gwen tells him to turn into Diamondhead even though Ben has not yet learned how to switch from one alien to another.
  • When Malware grabs Feedback, his Omnitrix symbol has a red outline for a frame.
  • When 11-year-old Ben twists the Omnitrix's dial before attacking Malware, the white tubes overlap the dial.

Showdown: Part 2[]

  • During the recap of the previous episode, the "Previously on Ben 10: Omniverse" text has "Ben 10" as one word.
  • When Galvan B was destroyed in the previous episode, several cracks erupted through the moon in sequence before it exploded in segments. When it is destroyed in this episode, several cracks are formed simultaneously, and it explodes in one go.
  • The same side of Galvan B is shown in space and on Galvan Prime, when the other side of the moon should be seen on Galvan Prime.
  • When NRG, Rook, and Azmuth land after destroying the rocks, the handle on NRG's containment suit is missing.
  • Right as Ben points to Azmuth saying "Seriously? You're the smartest guy in three galaxies," the Omnitrix is missing from his wrist.
  • In one shot, Max's eyes are not in the correct order.
  • In one shot, Max's eyes don't match.
  • While the team is in the Proto-TRUK, Rook has the Proto-Tool on his shoulder, even though it had already been absorbed by Malware.
  • Before Ben and Kevin drop from the Proto-TRUK, Azmuth is missing.
  • When Ben and Kevin fall, Kevin has no hair on his chin.
  • Throughout the episode, Way Big's cheek eyes are colored like the metal tubes encasing them.
  • When Malware knocks down Way Big, the red on his cheeks is white.
  • At one point during the flashback with Ben and Azmuth, Azmuth's feelers are missing on his head.
  • When Feedback appears out of Malware's head, his fingers are round instead of cubed.
  • When Feedback looks at Malware, his back has a green stripe.

Tummy Trouble[]

  • When Perk Upchuck eats the tigers, he has five tongues.
  • When Ben intends to transform into Jetray, the Omnitrix buckle of the seatbelt in Proto-TRUK is in front of his right hand.
  • When Big Chill is in the Proto-TRUK, his seatbelt is missing.
  • When Ben is being surrounded by Perk Gourmands, several of the Perks are improperly layered on top of others.
  • When the Murk Gourmands are on the Incursean ship rescuing the Queen, one of the Gourmands has green eyes instead of yellow.
  • When the Gourmands and Attea are in the Proto-TRUK, two Gourmands temporarily have Omnitrix symbols.
  • Throughout most of the episode, Sergeant Cookmeister wears a medal with five spots on it. In one shot where Sergeant Cast Iron stands beside Rook, his medal only has three spots on it.

Store 23[]

  • In one shot, Feedback has plugs on his hips.
  • When the Chicken Alien faces the counter to order, there is an emblem on the rhombus where his two sashes meet. When he walks towards the door, the emblem disappears. It reappears while he and the Cow Alien are being sucked through the pinwheel aurora.
  • When Ben peers through the window to see Blarney T. Hokestar, his head is smaller than usual.
  • While Ben 23 wears the Omnitrix on his left wrist, the billboard for the Secret of the Hero Watch movie features the watch on his right wrist.
  • When Ben 23 recognizes Spidermonkey as Mr. Monkey, the closed captions on the Ben 10 Omniverse: Aliens At War DVD call him Mr. Mucky.
  • After the bomb detonates, Ben Prime's Omnitrix is not drawn correctly.
  • In one shot, Electricyeti's crest is incorrectly colored as blue, instead of being yellow.

Vilgax Must Croak[]

  • At one point, Attea's eyes are white instead of the normal yellow color.
  • Vilgax says that he "would have perished with the rest of them" if he had been blasted into space. That is untrue, he said himself that he could survive space for a short period of time, and he survived getting blasted into space for a long period of time in the Original Series.
  • When Rook and the other Plumbers go up the elevator, the logo on the back of Rook's armor is all black.
  • When Molly Gunther shows Ben the hologram that reveals what happened to Vilgax, her hair is different.

Ben Again[]

  • When Diamondhead fights Eon, the sky is yellow instead of blue for an instant.
  • When 16-year-old Ben (in 11-year-old Ben's body) transforms into Ditto, the Omnitrix dial doesn't pop up despite making the sound of doing so.
  • When all of the Ditto clones are gathered, one of the clones is missing a bump on his wrist, while another is missing one on his shoulder.
  • When Gwen, Rook and 11-year-old Ben talk in Rook's truck, Rook's mouth disappears for a moment.
  • When Ben, Gwen and Rook run away from Pakmar's shop, the board of the shop reads "Packmar's Fine Teas".
  • In one shot, the Omnitrix has the prototype's faceplate.
  • 11-year-old Arctiguana (in 16-year-old Ben's body) says "I finally got an alien I know how to use" to Gwen, even though he had already used Diamondhead and Wildmutt.
  • When 16-year-old Ben (in 11-year-old Ben's body) transforms into Clockwork, the rim of the Omnitrix symbol is gold instead of silver.
  • The closed captions on the Ben 10 Omniverse: Aliens At War DVD misspell Maltruant's name as "Maltruent".

Special Delivery[]

  • In one shot, the symbol on Thunderpig's belt is missing.
  • When Way Big knocks Dr. Animo with a truck, the sky is blue instead of yellow.
  • When 15-year-old Ben transforms into Big Chill, his Omnitrix makes the sound that usually comes from his current Omnitrix instead of the original Omnitrix.
  • One of the incidents that made Mr. Baumann hate Ben is shown to have taken place during Alien Force. However, in The Purge, Baumann said he hadn't seen Ben since the latter was 12 years old.
  • After Mr. Baumann puts on Ben's hat, and the screen pans out, the Omnitrix is missing from Ben's wrist.
  • When Ben chases the Kraaho, the "10" symbol on his T-shirt is black.
  • When Humungousaur swings Sunder's axe, the colors of his Omnitrix symbol are inverted.

Rad[]

  • When Rook goes searching for Pax, the Galvan tech symbol on his back is all black.
  • Eye Guy's sight from his perspective is as if his eyes were clumped together on the body of a small creature. A view of all over the room should be visible from his perspective.
  • While the Incurseans are running away from the Gracklflint after it is let free, one is not wearing his helmet while the other three are. In the next shot, when they confront Rad, all four are wearing their helmets. In the following shot where they run from Rad's blaster, the one that was originally not wearing his helmet is no longer wearing his helmet again (while the other three are).

While You Were Away[]

The Frogs of War: Part 1[]

  • When the aliens fly out of Undertown, there is a man behind the policewoman wearing a red jacket. In the next shot after the policewoman screams "Aliens!", the man's jacket is now green.
  • The citizens of Bellwood are surprised that there are aliens living among them despite previous episodes such as A Jolt from the Past showing aliens mingling with humans openly.
  • When the Incursean ships are seen flying towards Earth while Milleous attempts to make an announcement to the planet, the hexagonal shield is not visible.

Blukic and Driba Go to Area 51[]

  • When Ben selects Diamondhead, the icon selection screen doesn't appear.
  • In Ultimate Alien, Colonel Rozum looked like he was in his fifties but in this episode, he is seen as the boss of the barely adult Max Tennyson. More than 40 years later, he looks almost exactly like his younger self.
  • In Ultimate Aggregor, Colonel Rozum had no idea who Professor Paradox was, but in this episode, in the flashback, he already knew him.
  • Professor Paradox shouldn't have been working for Area 51 since his experiment had caused him to be stuck out of time in the 1950s and since Max is just barely an adult, the events of the flashback would have been in the 1960s (Max was 17 during the flashback in Moonstruck which was set in either 1961 or 1962).
  • Previous flashbacks to Max's youth showed him with brown hair. In this episode, his younger self's hair is black.

Universe vs. Tennyson[]

  • In multiple shots, Magister Patelliday's lure has the same coloration as his skin.

  • In a sequence of the fight between Mutant Kevin and Swampfire, the right arm corresponding to Astrodactyl is missing.

  • Rook uses a contraction in the episode (specifically "would've").
  • In one shot, Rook's hands are white.
  • After Rook buys a meatball sub for the first time, the seatbelt buckles in the Proto-TRUK have their colors reversed.
  • Fistrick, when wearing his spacesuit, only has one spike on each shoe. However, when it breaks apart, he is wearing his regular four-spiked shoe. When the spacesuit breaks apart, the spikes reappear. This error is likely caused by his model sheet, which also does not contain the extra three spikes on each shoe.

  • Ben knew that Plumber Headquarters was an underground space station in Outbreak, but in this episode, he acts as if he is learning it for the first time.
  • When Rook talks to Bryk, he mentions Morty as being the same species. However, Bryk and Morty look nothing alike.
  • When Upgrade hacks the Plumber Headquarters' terminal and the camera faces him, the shot is pulled out farther than it should be, so the ends of the terminal model are visible and reveal when they cut off.
  • Neither Cannonbolt nor Fistina are shown saving Kundo.
  • Skurd is not present on Cannonbolt's Omnitrix.

Malgax Attacks[]

  • When Ben is playing his game at the beginning of the episode, he states it being a sandbox-based game, however, it is shown to be a game similar to Galaga.
  • The old Mr. Smoothy cup design can be seen in the Skipper.
  • In one shot when Azmuth inspects Skurd, part of the Omnitrix's rim is green.
  • Before transforming into Ultimate Albedo, Albedo's shirt is black with red stripes, instead of red with black stripes. This error occurs again when he transforms into Ultimate Rath.
  • When Albedo briefly transforms into his Galvan form, he is a red saturated Azmuth rather than his actual Galvan form.
  • Skurd states that, since he is a single-celled organism, a teleporter would rip him apart upon reconstruction. However, he did not show this fear in Stuck on You.
  • When flying up to Vilgax's ship, Skurd has the Omnitrix symbol on his head while he is on Luhley, despite not being attached to the Omnitrix at the time. This error occurs again when Vilgax pulls Skurd off of his face.
  • Gravattack is once again bigger than usual.
  • When Albedo's new Ultimatrix is disabled by Azmuth, he is unable to transform into an alien form even though he gained the ability to transform into aliens without an Omnitrix in Double or Nothing.

The Most Dangerous Game Show[]

  • At the start a Kaosseffexx Ultimasauria can be seen on TV, it has the Nemetrix collar when it shouldn't.
  • When Zenith teleports almost every woman Ben met, Myaxx is shown to be teleported twice.
  • Jennifer is not seen again after she is poofed away.
  • Rayona is eliminated and poofed away, but in the next group shot she is there again in place of Rook Shar.

  • In a scene in the Null Void, the first strap of Kai's left leg does not connect to her shorts.
  • In another scene in the Null Void, Kai's hip is smaller than it is supposed to be.
  • When Ben finds the girls by surprise in the Null Void, Subdora is seen with pants instead of shorts.
  • Helena Xagliv is in the group shot when Ben returns from the Null Void to confront Zenith, even when she shouldn't be there. Fistina is also there even though she was not in the game at any point.

The End of an Era[]

  • All throughout this episode, Ben 10,000's shirt is depicted as having an additional vertical green stripe down the middle of the back, in addition to the front stripe, when in all prior appearances it was just the front stripe present on his shirt.
  • One of the Plumbers who escorts Animo to his cell is shown in the background of the next shot.
  • In two shots, Subdora's tail is off-center.
  • In the end credits, Atomic-X's name is misspelled "Atomix X".

A New Dawn[]

  • George Washington states that "Plumbiers" is the French word for "iron workers". However, this is false; the word is not the translation of "iron workers" (ferroniers) nor "plumbers" (plombiers).
  • When Chromastone jumps after the Annihilarrgh, his Alien X arm is missing.
  • In one shot, Chromastone's upper arms are magenta like his lower arms.
  • When talking to Skurd, Chromastone's chest is colored green like his jumpsuit.
  • During the mass alien transformation scene:
  • When Feedback is about to launch his final attack on Maltruant, there is a rendering error in the right side of the scene.
  • When Feedback reverts back to Ben, Ben is seen with Alien X's left arm. Not only is it missing its standard white rim-lighting for most of its screentime, but its presence and retraction imply that it was brought out by Skurd while containing the Annihilarrgh's explosion but was not animated.
  • When Skurd deactivates Alien X's left arm, Ben's arm is missing its shading.
  • As Ben and Rook watch the formation of the universe, the Omnitrix is missing.
  • The Slime-Biot cage is open before the Contumelia opens it.
  • Towards the end of the episode, parts of Rook's Proto-Tech Armor are blue instead of black.
