- In many shots, the Omnitrix's dial faces the wrong way.
- For a brief frame before Ben is crushed by the boxes that were toppled by the Mutant Frog, the Omnitrix is green, even though it's supposed to be in Recharge Mode.
- When Security Guard #2 tries to stop Animo, he is voiced by Paul Eiding, but in when he grabs the walkie-talkie to report Animo, he is voiced by Dee Bradley Baker.
- When Ben hits the Mutant Hamster with a football and begins to lead it away, a black line can be seen on the back of Ben's shirt for a split second.
- When Ben turns into Four Arms, he doesn't activate the Omnitrix. The dial just pops out.
- When Four Arms jumps towards the Mutant Parrot, for a split second, the black line on his leg is not colored in.
- When Four Arms grabs the Mutant Parrot, his Omnitrix symbol disappears for a few seconds.
- When Ben falls from the sky as Four Arms, he creates a large crater, but when the Omnitrix times out and Ben turns back into his normal form, the size of the crater is too large for Four Arms to create.

The Krakken[]
- In many shots, the Omnitrix's dial faces the wrong way.
- When Gwen gets splashed with water by Ben, she gets wet, but when she turns back, she is no longer wet.
- When the gang walks to the dock, Ben says that he saw a lake monster with red glowing eyes, but the Krakken has blue eyes.
- After Ben scares Gwen with the bait worms and puts them back in front of him, the bucket is shown behind Ben in the next shot.
- While Gwen walks away from Max and Ben, she vanishes before reaching her destination.

Hunted[]
- When Ben grabs a wrench in the warehouse, the colors on the Omnitrix's dial are reversed.

- After Max throws the junk at Kraab, the Omnitrix appears on Ben's right wrist.
- When Tetrax knocks Diamondhead into a minecart, for a split second, the minecart is not drawn in and Diamondhead's upper half is cut off.

- When Ben and Kevin are stealing the Sumo Slammers game from the warehouse, two police officers come in to stop them. Kevin defeats one but the other one just disappears from the scene.
- Heatblast manages to stay on the train by melting a handhold in it. When he reverts, Ben is still holding it, and the metal is somehow cool enough that he is not burned.
- In one shot, the collar part on Gwen's shirt is up to her face.
- When Ben falls on the Rust Bucket, the Omnitrix is red and white.
- When Ben is about to turn into Four Arms, the floor of the Rust Bucket looks different.

- When Ben is making funny faces at the kid in the car next to him, the kid eats a burger that has a brown bun and red patty, but when he sticks out his tongue, it's all green.
- When Stinkfly laughs and slides across the floor, his tail is turned around instead of being upside down.
- In one of the photos, the cat logo on Gwen's shirt is missing.
- The "This is It!" sign seems to just be floating in the air.
- For split second, Four Arms' lower stripe is white.
- When Ben places his hand on the rubber ball, the Omnitrix's dial faces the wrong way. This error occurs again when the Megawhatt first appears.
- Gwen's model is seen on the top layer in one shot.
- When Upgrade stops running from the Megawhatt, there is no manhole next to his feet. In the next shot, however, there is one next to his feet.
- Before Upgrade changes back, there are four green lines down his back, even though there are only supposed to be three.
- When Ben is grabbed into the truck, he is grabbed from the left, but he sits on the right.
- When the Megawhatts flatten all the tires in town, in all the next scenes with vehicles in them, none of the tires are flattened.
- The Omnitrix activates and the dial pushes out without any interaction from Ben prior to the fight at the dam.
- Near the end of the episode, when Ben gets pranked, the Omnitrix is missing from his wrist.
The Alliance[]
- In some shots, the Omnitrix's dial is white and green instead of grey and green, and is drawn incorrectly.
- After Max is attacked, Four Arms carries him and Gwen to the hospital several blocks away as Four Arms, leaving the Rust Bucket at the gas station. However, when Max is discharged and is leaving the hospital with Ben and Gwen, the Rust Bucket is parked outside the hospital.
- When Ben activates the Omnitrix in an attempt to get an alien that can help Max, the core is popped up. When Rojo senses it and zooms in on this, the core is lowered again, and Ben hadn't selected an alien.
- When Rojo breaks into the hospital through the window, pieces of glass can be seen on the floor, but when Ben and Gwen start running away, the glass is gone.
- In many shots, the Omnitrix's dial faces the wrong way.
- During XLR8's fight with Rojo, a blue police car can be seen. When XLR8 dashes around Rojo, the car is gone. Later, the car reappears, but is now black.
- When XLR8 punches Rojo three times, his hands are human in appearance.
- When the paramedics arrive to the battle, in the shot after Rojo jumps on XLR8, the ambulance's left back door is missing.
Lucky Girl[]
- In some shots, the Omnitrix's dial faces the wrong way.
- In one shot, Gwen's shoes are all white with no blue on them.
- In some shots, the Charms of Bezel are missing from Hex's sash.
- When Hex revives in the ambulance, the charm to the right of the glowing Reincarnation Charm is the Luck Charm.
- When the trio are at the restaurant, the Luck Charm's top line doesn't glow.
- For a split second, the back of Lucky Girl's hair is missing.
- In one shot, the top of Wildmutt's lips is miscolored orange.
- When Ben selects Four Arms, the Omnitrix's core is grey.

A Small Problem[]
- In some shots, the Omnitrix's dial faces the wrong way.
- In one frame, half of Grey Matter's face is covered by the background.
- Despite Howell clearly covering Grey Matter with his hands, Grey Matter somehow manages to get in his shirt.
- Before Ben changes into Grey Matter, he is wearing his swimsuit. When he changes back to normal near the end of the episode, he is wearing his regular clothes.
Side Effects[]
- Joe is first voiced by Steve Blum at the beginning of the opening scene, but when Clancy shows himself, Dee Bradley Baker voices Joe instead, with Steve Blum voicing Bob. Joe is voiced by Steve Blum again when Clancy uses his wasp swarm to grab him.
- In many shots, the Omnitrix's dial faces the wrong way.
- When Max first talks about his San Ju Yien Pien remedy, his hair has inverted colors.
- When the Tennysons split up the hallway as they're exploring Clancy's building, Max walks off onto the wall as if it were the floor.
- When Four Arms says "Oops!", his mouth is open but does not move.
- The nuclear reactor has no employees and no one to stop the Tennysons from coming in.
- No one requires radiation protection, despite direct exposure to a nuclear core.
- In one shot when Ben tries to fend off the wasps in the Rust Bucket, the Omnitrix's dial is white and green instead of grey and green.
Secrets[]
- In many shots, the Omnitrix's dial faces the wrong way.
- In the picture of Four Arms and Stinkfly on the Chimeran Hammer, they are not wearing their gloves.
- The cat logo on Gwen's shirt is a different shade of blue in one shot.

- During various shots before XLR8 fights the Wigzelian Org Beast, the Omnitrix symbol looks bigger than usual.
The Big Tick[]
- At the beginning of the episode, front side of the Rust Bucket has missing lines.
- When the Leader Alien says "And an Arburian Pelarota to boot", the Ben 10: The Complete Season 2 DVD's closed captioning says, "And an Aburian Pelorota to boot".
- In many shots, the Omnitrix's dial faces the wrong way.
- Gwen's cat logo on her shirt disappears several times in the episode.
- When Ben jumps on the Interpreter Alien's platform and knocks him into the Great One, it never shows any physical damage, seeming as he just phased through it.
- In some shots, the Omnitrix's dial is white and green instead of grey and green.
- After Ben fails to destroy the Great One as Four Arms, he says, "Okay, one down, nine to go!", meaning he was going to try all of his other aliens (not counting Cannonbolt, who he considers to be useless). However, he doesn't use Grey Matter or Stinkfly.

- When Kevin and Ben jump, the Omnitrix is green, but is then red again.
- In one shot, Ben's shackles are on his left leg instead of his right leg.
- When Kevin spits the Stinkfly goo at Ben, it hits him on his back. However, when Ben is bound by the goo, he faces Kevin.
The Galactic Enforcers[]

- After Ultimos eats the chocolate, nothing is shown near his mouth but when the allergic reaction takes place, there is a chocolate smudge beside his mouth.
- When Vulkanus picks up the container of iron ore it is grey, but when it shows him and SixSix going towards the ship, it is the same type of yellow container Tini was trapped in.
- When Ben wears his Galactic Enforcers uniform, the Omnitrix is covered by a glove, but when he uses it to turn into XLR8, the Omnitrix is over the glove.
- In one shot, the colors of Ultimos' emblem are reversed.

Dr. Animo and the Mutant Ray[]

- In one shot, Max is not wearing a life jacket.

- For two frames, Gwen's lower half is cut off.
- At the end of the episode, Ben states that if he broke the Omnitrix's faceplate again, he could possibly turn into a combination between XLR8 and Ghostfreak (XLR-Freak). However, this would not be possible as Ghostfreak escaped from the Omnitrix several episodes ago.
- When Ben fiddles with the Omnitrix at the end of the episode, in one shot, the Omnitrix's dial is drawn in an oval shape.
Back with a Vengeance[]
- For some reason, Ben's transformations are wearing the raincoat, despite the fact that the Omnitrix breaks down the clothes when Ben transforms.
- Heatblast does not burn the raincoat when using it, despite him burning everything he touches.
- When Wildmutt is wearing the raincoat, it is shown to be intact despite Diamondhead's shards causing a tear in the back.
- Gwen is surprised when Ben as Ripjaws changes into Stinkfly, despite not showing shock at Wildmutt being right next to her and changing into Ripjaws.
- When Vilgax throws the container at Kevin, it creates a breakage in the right side of the glass. However, in the next scene, when Kevin is shown to be firing crystal shards at Vilgax, the breakage is shown on the left side of the glass. Also, when Vilgax and Kevin exit that part of the Megacruiser, the breakage in the glass is shown to be bigger than it was when Vilgax threw the container.
- When Vilgax holds Ben atop the sky ride, the Omnitrix's dial faces the wrong way.
- The closed captions for the Ben 10: The Complete Season 2 DVD misspell Havoc Beasts as "Hyvex Beasts".
- Gwen doesn't have the grenades when Kevin is holding her.
- The container Kevin is carrying already had the Omnitrix before Kevin took it off of Ben.
- When Kevin pins Ben down with his foot and uses a device to tear the Omnitrix off of Ben's wrist, for a moment his foot looks green/blue diamond-like instead of reddish.
- When Kevin removes the Omnitrix from Ben's wrist, XLR8's silhouette has human-like feet.
Midnight Madness[]
- When Ben views Gwen's videotape, the Omnitrix's dial faces the wrong way.
- Gwen and Max watch Gwen's videotape from earlier when determining that Sublimino is responsible for Ben's actions. The video shows Sublimino facing the camera during his hypnosis routine. From where Gwen was standing, his back should have been facing the camera.
Ben 10,000[]
- In one shot, XLR8's face is miscolored below his lip.
- Ben asks Ben 10,000 about unlocking the other 9,990 aliens. However, at this point Ben had discovered 12 aliens, so he actually should have said 9,988.
- In various shots during the episode, the Omnitrix's dial faces the wrong way. This error also consistently occurs with Ben 10,000's Omnitrix.
- The charms on Gwendolyn's arm change places, from being three on the upper arm and two on the forearm to two on the upper arm and three on the forearm, in some shots.
- XLR8 grabs Ben and Gwen, but then he runs alone.
- When XLR8 first encounters Dr. Animo in Sector 1-A Centric, his visor is miscolored green.
- In one shot, Future Diamondhead's Omnitrix symbol has its colors switched.
- When Upgrade unmerges with the hoverboard, his Omnitrix symbol is missing.
- After Cannonbolt and Four Arms get knocked down by Vilgax, Cannonbolt's fingers have black lines around it like Ben 10,000's Cannonbolt. In the next shot, the lines are gone.
A Change of Face[]
- When Charmcaster in Gwen's body asks Ben how the Omnitrix works, the Omnitrix's dial faces the wrong way.
- When Charmcaster in Gwen's body gets off the truck to buy more ingredients, she leaves her things on the truck.
- When Gwen and Charmcaster speak in each other's bodies, their voice is of the host body, but when Ben and Gwen switch, it is not.
- When Charmcaster is in Gwen's body, the back of her hair is missing in one shot.
- When Ben and Gwen are trapped, Gwen's pants suddenly have back pockets, which are not usually seen.
- In one shot, the inside of Gwen's right sleeve is the same color as her skin.
- When Charmcaster's bag falls into the ocean, the strap is shown to be intact, but when it's shown sinking into the water, the strap is broken.
Merry Christmas[]
- In many shots, the Omnitrix's dial faces the wrong way.
Benwolf[]
- In one shot when the Werewolf is fighting Benwolf, he is wearing Ben's shirt.
- When the Werewolf scratches the Omnitrix, the dial just glows yellow and starts rotating, but when Benwolf recalls what happened, the flashback shows the dial as a mixture of the inactive and active modes, and the tubes are yellow instead of white. After Benwolf recalls the flashback twice, the tubes are shown white.
- When Ben reverts to human from Benwolf, the Omnitrix is in Active Mode.
- At the end of the episode, before Cannonbolt gets out of the cave, Kai's shirt is purple instead of peach.
Game Over[]
- When Ben hands Gwen the game controller, the Omnitrix is missing from his wrist.
- When Ben and Gwen begin to play Sumo Slammers Smackdown, the Omnitrix's dial faces the wrong way. It also almost always faces the wrong way when Ben and Gwen are trapped in the game.
- In a couple of shots, Cannonbolt's back is white instead of yellow.
- When Ben finds the Four Arms/Upgrade icons, he has to activate the Omnitrix to use them, but when he finds the Cannonbolt icon, he changes instantly.
Super Alien Hero Buddy Adventures[]
- In some shots, the Omnitrix's dial faces the wrong way.
- In one shot, the globe featuring the logo of Planetary Studios reads "Netary Studios" instead.
- When Tim Dean is signing the pictures, he signs Doggy Buddy's picture on the stack of paper and then takes it off the stack. When the paper is on the stack, Doggy Buddy has a white mouth, but when Tim takes the paper off the stack, it has a black mouth.
- In one shot, Four Arms has five fingers.
- After saving the girl, Kangaroo Kommando has no "K" on his head.
- While Ben falls after reverting from Stinkfly, all the black parts on the Omnitrix are grey.
Under Wraps[]
- In many shots, the Omnitrix's dial faces the wrong way.
- Wildmutt's lips are orange in a few shots.
- When Gwen goes out of the machine, the sides of her hazmat suit are miscolored.
- When the Omnitrix scans the Mummy's DNA, Ghostfreak's silhouette can be briefly seen even though he is no longer in the Codon Stream.

The Unnaturals[]
- Ben's pants are torn when he gets hung on a pole, but when he falls, his shirt is also torn.
- The word "League" is misspelled as "Legague" on Gwen's laptop.
- In a shot, before the Omnitrix recharges, it is first shown green. After a few frames, the Omnitrix goes back on Recharge Mode, but when Ben moves his left arm, it is shown green.
- Gwen's eyebrow is thinner for a bit in one shot.
The Visitor[]
- At the beginning of the episode, the Omnitrix's dial faces the wrong way. This error occurs again after Ben reverts to human from Upchuck.
- Ben somehow transforms into Four Arms without pressing down on the Omnitrix's dial.
- Xylene calls Upgrade a Glavanic Mechamorph, when his species is Galvanic Mechamorph.
- When Diamondhead says "You're not the boss of me", his back is completely diamond. He also is missing the black patch on his left pectoral which houses the Omnitrix, and is missing the Omnitrix, as well.
- In most shots when Max is on the roof of the Rust Bucket, the symbol on the shoulder of his Plumber suit is missing.
- In some shots, Upchuck has four tongues.
The Return[]
- In many shots, the Omnitrix's dial faces the wrong way.
